Send in Your New Board Contact Info . . .

PTA is a large organization. In order to ensure the best communication possible, we ask that every unit and council submit information regarding their key contacts in May.

In order to make this process as easy as possible, you may submit your organizations information via one of these options . . .

Option 1 — Complete the Online Form
(scroll down to the bottom of this page)

Councils and units can complete the online form below (scroll down to the bottom of this page and begin).

Upon completion, the information will automatically be submitted to First District PTA.  You will receive an email confirmation that will include the info that was submitted.  This will give you an opportunity to double-check the information.  Also, for units, this email can easily be forwarded to your Council leadership.

(NOTE: You may wish to print out the fillable form to use as a guide before going online to complete the online form.)

Option 2 — Complete the Form in a Fillable Word Document

Print out the fillable form in Word. Complete the form, save it (you may wish to name the file with your organization’s name), and then forward it to First District at, and, if you are sending info for a unit, please also send it to the appropriate person in your Councll.

Option 3 – Complete Excel Worksheet

If you prefer to provide the information in an Excel worksheet, please send an email to and request the excel worksheet for your organization.  Then complete it, save it and send it in to First District, or, if you’re a unit, send it to the appropriate person in your Councll.

Just remember, no matter what you choose, please be sure to send in your current contact information by May 1st – or as close to May 1st as possible.  We hope to receive everyone’s contact information in record time this year!
